Orion StarBlast Telescope Lending Policy

The Orion StarBlast Telescope was given to the Smithfield Public Library by THE CACHE VALLEY ASTRONOMICAL SOCIETY. It is a nice addition to the Library.  Something that everyone can enjoy using and nurture an interest in the night sky.

Warning: Do not look at the sun with the telescope; doing this can blind the user and damage the telescope. The library is not responsible for any damages a patron inflicts upon oneself or the telescope.

The Library’s telescope may be checked out by a patron of the Library who is 18 years of age or older and in good standing.

The patron will sign a Telescope Borrowing Agreement each time they check out the telescope.

The telescope will be checked out for one week and must be handed directly to a staff member when returned.

The overdue fine for the telescope is $5.00 per day. If the telescope is not returned within two weeks of the due date the patron will be charged $350.00 to cover replacement costs.

If the telescope or any of the materials that are included, are damaged, a fee will be determined by the library based on the cost of repairing or replacing the telescope and materials, and will be charged to the patron.

The patron is not permitted to lend the telescope to anyone while the telescope is in their possession. The telescope is their responsibility during the time it is signed out in their name.

Children must be supervised at all times while using the telescope.

All parts of the device and the books included must be returned together. Patrons will be charged for missing items.

Please treat the telescope with care, Keep the telescope covered when not in use. Keep it clean, dry, dust-free and safe from liquids, extreme temperatures, and from being dropped. All covers should be kept attached to the Telescope at all times. Do not attempt to clean the telescope with any chemicals or liquid or store the telescope outdoors. Keep the dust caps on the front of the telescope and on the eyepiece when not in use. Make sure to turn off the viewfinder when not using it.
To transport the telescope, place your forearm under the telescope tube when it is in a level position and lift, holding the upright support against your side. When in a car, simply place the scope on a seat and use a seatbelt to secure it. The lap belt goes across the base, and the chest belt should cross the tube.

Digital Device Policy

Purpose of Lending Digital Devices*

Increased Content: The digital Devices provide access to numerous content titles (books, audiobooks, movies, apps) that cost and space limitations would otherwise prohibit.

Increased Accessibility: Digital devices provide accessibility features (test-to-speech & large print) that may enhance content for patrons.

Technological Literacy: Increase opportunities to explore, learn and access digital information through dedicated digital devices.

Kindle Lending Guidelines

Kindles will only be checked out to patrons 18 years or older who have a valid Smithfield Public Library account AND a valid Utah issued identification card.
Patrons must sign a user agreement. By signing patrons accept full financial responsibility for the Kindle and agree to pay all costs associated with damage to or theft/loss of Kindle and /or Kindle accessories while checked out.
Kindles must be checked out and returned to a staff member at the Circulation Desk at least 30 minutes before closing.
The checkout period will be 14 days.
No renewals are allowed. All Kindle components must be present upon return for the library to consider the item checked in.
Late fee of $5.00/day. No grace period.
Because Kindles are fragile, if found in the book drop the patron will be charged a $25.00 fine.
Patrons may request holds.
Holds not picked up after 5 days will be reshelved.
Replacement costs are $300 for Kindle and content, $50 for cover or carrying case and $45 for the charger or adapter.

Hotspot Policy

This agreement details the terms and conditions of borrowing the Smithfield Library’s circulating hotspot devices (hereafter referred to as “devices”). The devices covered by the agreement are Jetpack 8800L. This policy is subject to change. The official copy is posted on the library’s website.

Requirements for Use of Devices

Borrower must be eighteen (18) years of age or older.
Borrower must have a Smithfield Library account in good standing.
Borrower must sign this Borrower Agreement.
Borrower recognizes that devices are remotely managed by the Smithfield Library and service will be disabled if devices are not returned.

Borrower Responsibilities

The Borrower is financially responsible for loss, theft, or damage to the devices, up to the full replacement cost of the devices.
The Borrower is responsible for returning the devices and accessories in the same order and condition as received. This includes NOT changing settings or otherwise altering the devices. If a device is lost, stolen, or damaged while on loan, the Borrower is responsible for reimbursing the Smithfield Library for repair or replacement costs up to the full cost of the devices, as determined by the Library Director.
The Borrower is responsible for keeping the devices secure at all times during the loan period.
The Borrower is responsible for immediately informing a library staff member of any problems with the devices.
The Borrower is responsible for any use by minors of the device while the device is in the Borrower’s possession.
The Borrower is responsible for adhering to the Library’s Internet Use Policy, Wireless Network Terms of Use, and this technological device policy. All policies may be found on the library’s website.

How to Borrow Devices from the Library

Approach the circulation desk and ask to borrow a hotspot device and give them your card number (Patron ID) or last name so it can be checked out to you.
Devices and their accessories are available for use in the order hold requests were received then by a first come, first served basis.
Devices circulate for a period of two (2) weeks. One renewal is allowed as long as there are no holds on the device.
A maximum of one device named in this policy may be checked out per family at any given time.

How to Return Borrowed Devices:

Devices must not be left unattended at the circulation desk or in the book drop; they must be handed directly to a library staff member.
Devices are considered lost if not returned within 72 hours of their due date; the Borrower will be billed for the repair or replacement if a device is returned damaged or with missing parts.
Devices will be inspected after check-in and removed from the Borrower’s account. The Borrower understands charges for lost or damaged devices may be applied up to 72 hours after returning the device to the library.
The Borrower agrees to pay Smithfield Library upon demand for the costs to repair or replace devices or their accessories. Substitutions are not authorized as a replacement for lost or damaged devices.
